Records Requests

Steps

1. Complete the following DHS Records Request Form:

2. If you are requesting records for someone other than yourself or your child, print and fill out the following DHS Release of Information (ROI) Form: 

ROI-Authorization for Disclosure of Health Info. Confidential Info

3. Make a copy of your ID (front and back) to verify identify if you are having your records mailed or emailed to you. If picking-up records in person, a copy of your ID will be needed at the time of pick-up. 

A copy of your ID is not needed in the following situations:

-Social Security Disability Determination requests
-Attorneys retained to represent clients
-Foster care background check requests
-Records for records to support coordination of care with a signed release of information

4. Submit your completed DHS Records Request Form, Release of Information, and a copy of your driver’s license via one of the following means:

Fees

Eau Claire County DHS does not charge a fee for records requests. 

Processing Time

The Department will attempt to fulfill all records requests within 30 days of receipt.

Request Form.
